About Your Visit

Thank you for choosing Fair Lawn Diagnostic Imaging Center. In this section you will find several resources that will help you prepare for your exam.

We will usually call you the day before your test to reconfirm your appointment. Because we know your time is valuable too, whenever possible, if we are running late, we will call you to let you know so that you can adjust your schedule accordingly.

Naturally, if, for any reason you are unable to keep your appointment, we want you to contact us as soon as you know, so we can reschedule you for a time that is more convenient for you.

Preparing For Your Exam

Your preparation prior to your test will vary, depending on the specific test you are having. To determine what preparation is required on your part prior to arriving for your test, please click on the appropriate procedure below.

The Day of Your Appointment

Please arrive at least 15-30 minutes prior to your appointment time unless otherwise specified when scheduling your exam. Also, bring the following items with you to your appointment:

  • Photo ID
  • Insurance Card(s)
  • Prescription from referring physician
  • Letter of authorization and/or referral if required
  • Co-payment
  • Driver, if you are going to be sedated
  • Relevant prior imaging studies and/or reports
Patient Forms

Before your appointment with us, quickly and conveniently access patient forms from our center. Please bring your completed forms with you at the time of your visit.

Fair Lawn Diagnostic Imaging Centers have been designated as Imaging Centers of Excellence by the American College of Radiology (ACR) in Magnetic Resonance Imaging (MRI), Computed Tomography (CT), Mammography and Breast Ultrasound.
