For your convenience, a list of the insurance plans we participate with is provided here. We are a participating provider with most major medical insurance companies and health plans. We will submit your claim, and any secondary claims, under the appropriate benefit coverage, including workers’ compensation, auto insurance and personal injury, on your behalf.
For some tests, many carriers require prior authorization. If your physician scheduled your appointment, he or she should have obtained this authorization at that time. We recommend that you contact your insurance company to confirm that he or she did.
You will be responsible for any co-payment required at the time of your test.
If you are self-insured, we will expect payment at the completion of your tests, by either cash or check or most major credit cards. Where required, we will work with you to arrange a payment plan.
