To arrange a test for one of your patients, you or a member of 
your office staff can either call us at 201-794-3132 or Request an Appointment online.
Depending on the specific services required, we will usually be able to schedule your patient’s test for within 24 to 48 hours, in some cases, on the same day.
To enable us to schedule and complete your patient’s tests as quickly as possible, it is important we receive the following information at the time of scheduling:
- Patient’s full name, date of birth and social security number
- Patient’s home and, if any, work telephone number
- If the patient has medical insurance that will cover the testing modality you are prescribing, the name and phone number of the insurance carrier and the patient’s insurance ID number
As you know, some insurance carriers require pre-authorization for certain types of testing. You should secure such pre-authorization when necessary and furnish it to the patient along with your prescription and referral slip.
Note that you should advise your patient to:
- Contact our office (as soon as possible), time permitting, to confirm their appointment and to provide any additional or missing information.
- Arrive at our office 20 minutes prior to their scheduled test to complete and sign any required forms.
